The Columbia-Suicide Severity Rating Scale (C-SSRS) is a simple questionnaire designed to assess and prevent suicide. It helps identify if a client is at risk for suicide, the severity and immediacy of the risk, and also how much support the person needs. It asks about suicidal ideation and behaviors and other details about suicide attempts.
