Patient Health Questionnaire-2

The Patient Health Questionnaire-2 (PHQ-2) asks about a person's depression like the PHQ-9 and PHQ-8. The PHQ-2 is made up of the first two questions of the PHQ-9. It is intended as a quick depression screener. Positive tests should be followed up with additional testing like the PHQ-9.

Scoring and interpretation

Answers are scored on a 4-point Likert scale from 0 for "Not at all" to 3 for "Nearly every day." Scores are summed and total scores range from 0 to 6. A score of 3 or greater suggests a positive screen for depression and more follow up testing is recommended.
