Patient Health Questionnaire-8

An eight-item version of the Patient Health Questionnaire-9 (PHQ-9) for depression. It also measures the severity of depressive disorders. The one question missing from the PHQ-8 is the final question on the PHQ-9, "(How often have they been bothered by the following over the past 2 weeks?) Thoughts that you would be better off dead, or thoughts of hurting yourself in some way?"

Scoring and interpretation

Answers are scored on a 4-point Likert scale from 0 for "Not at all" to 3 for "Nearly every day." Scores are summed and total scores range from 0 to 32. A score of 10 or greater may be considered major depression and a score of 20 or greater may be considered severe major depression.
